Course objectives

At the end of the course student should be able to prescribe representatives of individual drug classes in the form of individually prepared pharmaceutical preparation and bulk drugs. The student should be familiar with pharmacological profiles of therapeutic and adverse effects of particular drugs with an attention paid to common drug-drug interactions.

VLFA07212c - Pharmacology I - practical lessons

General medicine

Autumn semester 2018
1^st week
Topic: INFORMATION ON DRUGS
Course content: Organisation of practical courses and lectures, system of evaluation. Repetition of basic pharmacologic terminology. Sub-branches of pharmacology. Sources of information concerning drugs and recommended textbooks. Work with relevant databases (Brevíř, SÚKL, EMA, AISLP), ATC classification.
Self-studying preparation for practical lessons: Materials for the Lecture No. 1 and the Chapter 1.1 in the textbook Nováková J. a kol. - Latin terminology, Pharmacological calculations with the use of the textbook for Practical lessons.
2^nd week
Topic: PRESCRIPTION OF MEDICAL PRODUCTS I
Course content: General rules of drug prescription. Prescription of the individually-made solid, liquid and semi-solid pharmaceutical dosage forms.
Self-studying preparation for practical lessons: Materials for the Lecture No. 2. Drug dosage forms from the textbook. Legislation of the Czech Republic related to drugs and their prescription (selected section of the Act No. 378/2007 Coll., on Pharmaceuticals and on Amendments to Some Related Acts).
3^rd week
Topic: PRESCRIPTION OF MEDICAL PRODUCTS II, ROUTES OF DRUG ADMINISTRATION
Course content: Brief characteristics of the individual drug dosage forms. Prescription of ready-made drug products. Clinical significance of the way of drug administration.
Self-studying preparation for practical lessons: Materials for the Lecture No. 3. Information in drug databases - SÚKL (www.sukl.cz), EMA and Drugs.com.
4^th week
Topic: PRINCIPLES OF PHARMACOKINETICS
Course content: Pharmacokinetic processes, parameters and calculations.
Self-studying preparation for practical lessons: Materials for the Lecture No. 4.
5^th week
Topic: PRINCIPLES OF APPLIED PHARMACOKINETICS
Course content: Pharmacokinetic case reports, simulation of plasma concentrations course, dosage optimalization and use of software tools, factors affecting course and value of drug´s plasma concentrations.

6^th week
Topic: PHARMACODYNAMICS
Course content: Mechanisms of drug effects - classification, examples. Dose-response curves.

Topic: ADVERSE DRUG REACTIONS
Course content: Classification and characteristics of adverse drug effects. Pharmacovigilance in clinical practice, sources of current information on drug safety. Databases of drug interactions. Pharmaceutical interactions (case reports).
Self-studying preparation for practical lessons: Materials for the Lecture No. 7, case report (drug incompatibility) materials in IS.
8^th week
Topic: FACTORS AFFECTING DRUG EFFECTS
Course content: Pharmacotherapy in childhood, in pregnancy, in breastfeeding women, in elderly. Pharmacogenetics.
Self-studying preparation for practical lessons: Materials for the Lecture No. 8. Pharmacogenetics - materials in IS.
9^th week
Topic: PHARMACOLOGY OF SYMPATHETIC NERVOUS SYSTEM
Course content: Drugs affecting sympathetic nervous system - classification, basic pharmacological characteristics.
Self-studying preparation for practical lessons: Materials for the Lecture No. 9. Pathophysiological bases - ANS, neurotransmitters (their synthesis and degradation), receptors of the sympathetic nervous system (alpha, beta) and parasympathetic nervous system (nicotinic, muscarinic) and their subtypes and organ distribution. Systems of second messengers. Pharmacodynamics, both direct and indirect mechanisms of action.
10^th week
Topic: PHARMACOLOGY OF PARASYMPATHETIC NERVOUS SYSTEM
Course content: Drugs affecting parasympathetic nervous system - classification, basic pharmacological characteristics. Spasmolytic drugs.

11^th week
Topic: GLUCOCORTICOIDS, DRUGS OF ABUSE
Course content: Repetition of basic pharmacological characteristics of glucocorticoids. Problems of drug abuse, doping and falsification of medical products.
Self-studying preparation for practical lessons: Materials for the Lecture No. 11, preparation of oral presentation should be uploaded to IS at least 24 hours prior to the practical lesson.
12^th week
Topic: PHARMACOTHERAPY OF DIABETES MELLITUS
Course content: Repetition of anti-diabetic drugs - basic pharmacological characteristics. Drugs used in therapy of specific and acute conditions in diabetology. Combinations of insulins, insulin regimens.
Self-studying preparation for practical lessons: Materials for the Lecture No. 12, preparation of oral presentation - presentation should be uploaded to IS at least 24 hours prior to the practical lesson.
13^th week
Topic: SEX HORMONES
Course content: Endocrine therapy, hormone replacement therapy, hormonal contraceptive drugs.
Self-studying preparation for practical lessons: Materials for the Lecture No. 13. Uterotonics and tocolytics.
14^th week
Topic: ANALGESIC DRUGS
Course content: Basic pharmacological characteristics of opioid and non-opioid analgesic drugs, antirheumatoid drugs and antiuratic drugs.
Self-studying preparation for practical lessons: Materials for the Lecture No. 14. Antirheumatoid and antiuratic drugs.
15^th week
Topic: ANAESTHETIC DRUGS
Course content: PRESCRIPTION TEST. Basic pharmacological characteristics of general and local anaesthetic drugs. Premedication prior to general anaesthesia.

1/ANALGESICS.
Opioid analgesics. Pharmacotherapy of pain. Opioid addiction therapy. Prescription and practical use of analgesic preparations.
2/NON-STEROIDAL ANTIINFLAMMATORY DRUGS, DRUGS USED IN GOUT, ANTIPYRETICS.
Mechanisms of action and classification of anti-inflammatory drugs. Drugs used in therapy of gout.
Getting ready for the next lesson: Central neurotransmitters and sites of drug effects in CNS. Basic psychical functions.
Day 2 (Tuesday)
3/ANTIDEPRESSANTS.
Classification and terminology of antidepressants. Review of their mechanisms of action, adverse effects, interactions.

4/THERAPY OF ANXIETY, HYPNOSEDATIVES.
Classification and terminology of psychotropics. Review of their mechanisms of action. Pharmacology of anxiolytics and hypnosedatives. Mechanism of action of benzodiazepines. Prescription of hypnosedatives and psychostimulants.

Classification, mechanisms of action, combinations of antibiotics and chemotherapeutics.

8/ANTIMICROBIAL AGENTS II., THERAPY OF INFECTIOUS DIARRHEAS AND UROINFECTIONS.
Drugs for the treatment of intestinal and urinary infections.

Day 6 (Monday)
REVISING TEST II (Overhead-test with subsequent discussion). Lessons 5 - 8.

10/GENERAL AND LOCAL ANAESTHETICS
Combined and balanced anaesthesia. Signs and stages of general anaesthesia. Inhaled and injected anaesthetics - classification and characteristics. Review of drugs used in premedication.

Getting ready for the next lesson: : Allergic reactions – causes and mechamisms. Revision of PNS pharmacology - β₂ mimetics, parasympatholytics. Glucocorticoids.Revision of blood coagulation.
Day 7 (Tuesday)
11/ANTICOAGULANTS AND THROMBOLYTICS.
Fibrinolytics, haemostatics.

12/H₁ ANTIHISTAMINES, TREATMENT OF BRONCHIAL OBSTRUCTION.
Classification and clinical use of H₁ antihistamines.Sites of action and review of drugs of bronchial obstruction and other drugs acting in respiratory tract.

Getting ready for the next lesson: : Drug dosage forms and external routes of drug administration, revision of drug prescription. FINAL PRESCRIPTION TEST (includes dermatology).
13/HYPOLIPIDEMICS, THERAPY OF OBESITY.
Therapy of hyperlipoproteinaemia and hypercholesterolaemia, review of drugs and their mechanism of action.
14/ANTIHYPERTENSIVE AGENTS.
Introduction, classification, diuretics, beta blockers, ACE inhibitors, Ca 2⁺ blockers, and other hypertensives.

Teaching methods

Drug classes specifics from the Syllabus are read firstly by the teacher and presented in case reports from real clinical praktické followed by discussion and training of drug prescription with the use of drug compendia and the computer database of the preparations registered in the Czech Republic (AISLP). Each student is asked to get ready as a homework to report on a specific pharmacological topic.

Assessment methods

The participation in practicals is obligatory and registered. To get credits a student has to pass 4 written tests: a) 1 on special drug presriptions (assessed individually by the teacher), b) 3 multiple-choice tests on knowledge of topics taught (more than half correctly answered questions required).
