Školení o bezpečnosti a ochraně zdraví při práci a požární ochrany
přímým nadřízeným zaměstnance na přiděleném pracovišti
Training on health protection and occupational safety at work and fire protection performed by
an immediate superior of the employee at the assigned workplace
I.
Zaměstnanec byl seznámen s těmito bezpečnostními předpisy
a zásadami pro:
pracoviště
pozice
 s provozním řádem pracoviště
 s riziky vykonávané práce a s opatřeními k jejich minimalizaci
 s obecnými zásadami poskytnutí první pomoci, včetně umístění a
vybavení lékárničky
 se způsobem hlášení pracovních úrazů a úrazů na pracovišti
(Informační list o úrazu se nachází v lékárničce)
 s umístěním a použitím hasicích prostředků, způsobem evakuace
 se zásadami evidence a používání osobních ochranných
pracovních prostředků
 s pracovištěm a pracovním prostředím
 se zásadami skladování
 se zákazy, příkazy a pokyny pro bezpečný pohyb na pracovišti
 s návody k obsluze následujících strojů, přístrojů a zařízení:
 s bezpečnostními listy následujících nebezpečných chemických
látek a směsí:
 pro výkon uvedené profese s příslušnou technickou normou,
vnitřním předpisem (organizační směrnicí, provozním předpisem,
…) případně jiným právním předpisem (zákonem, vyhláškou,
nařízením vlády) - uvede se název dalších:

The employee was informed about the safety regulations for:

 risks associated with the performed work and measures for
their minimising
 general principles of first aid, including the placement and
equipment of first aid kits
 the manner of reporting work injuries and accidents at the
workplace (the register of injuries can be found in the first aid
kit)
 placement and use of fire extinguishing devices, the method
of evacuation
 principles of record-keeping and use of personal protective
equipment at the workplace
 the workplace and the work environment
 storage principles
 prohibitions, orders and instructions for safe movement at the
workplace
 user manuals of the following machines, devices and
equipment:
 with Safety data sheets (SDS)
 For the exercise of the given profession the employee was
made familiar with the appropriate technical standard or other
regulation (law, decree, Government regulation - give the
name and number others)

Stvrzuji svým podpisem, že jsem byl řádně seznámen s výše uvedenými
předpisy a zásadami k zajištění bezpečnosti a ochrany zdraví při práci a
požární ochrany, seznámení jsem porozuměl a moje vznesené dotazy mně
byly vysvětleny. Zavazuji se probírané zásady a povinnosti při své práci
dodržovat a jsem si vědom možných důsledků pracovněprávních i
trestněprávních vzniklých při jejich nedodržování. Mé znalosti z těchto
předpisů byly ověřeny ústním pohovorem.

I hereby confirm with my signature that I have understood the topics of
the training and that I have been duly informed of the aforementioned
regulations and rules aimed at ensuring work safety and health
protection at work, that I have understood such information and that all
my questions have been answered. At the same time, I undertake to
observe the addressed rules and obligations during my work and I am
aware of the potential consequences in terms of labour law and in terms
of criminal law that could arise from a failure to observe these rules and
obligations. My knowledge of these regulations and rules have been
tested by means of a verbal discussion, with the result “passed”.
