The subject-matter of the summer vacation practical training of 4^th year students in Internal Medicine The aim is to obtain practical knowledge of the operation of an internal medicine ward and to check theoretical knowledge directly at the patient’s bed as well as in outpatient practice. It is necessary that the student should get acquainted with the doctors’ activities and with the activities of the nursing staff of the internal medicine ward of hospitals both in the inpatient and outpatient departments. 1. Students work at the patient’s bedside under the leadership of a house physician; they examine newly admitted patients, take their case records, make suggestions of the diagnostic examination procedures, and prepare the corresponding documentation. They make themselves acquainted with the printed forms and cope with the administrative procedures (decease reports, infectious diseases reports, and oncology reports). 2. They participate regularly in morning ward rounds in co-operation with the house physician. They monitor the development of the disease, take part in analyses, determination of diagnoses, differential diagnoses, and in therapy. They will be assigned 4 to 5 patients. 3. They will learn how to write case courses, summaries of diagnoses, treatment records, recommendations for domestic treatment, and discharge reports. 4. In addition to taking blood samples and applying injections, they will also cope with the introduction of gastric and/or duodenal probes, urine catheterisation, and withdrawal of material for examination. 5. They will take part in all of the more complicated examinations, e.g. bone marrow taps, punctures of nodes, liver, spleen, pleural and abdominal punctures, and in endoscopic examinations. They will have the possibility of independently examining urine including the deposit, blood count together with the differential, and/or sternal puncture. They will get acquainted with work at an ECG, X-ray and ultrasound unit. 6. They will learn to know all of the prescribed procedures used in applying blood transfusions. 7. They will get acquainted with work at an I.C.U. and at a differentiated care ward. 8. They will get acquainted with work at the admission ward and in the outpatient department and, if possible, they will learn to know the work in a local or industrial G.P.’s consulting room. 9. They will attend in two standby duties so that they may get to know the casualty procedures and the respective treatment methods. The students will keep records on the virtual activities to be acknowledged by the respective leading persons. The subject-matter of the summer vacation practical training of 4^th year students in Surgical Medicine During the summer vacation practical training, the students are to be acquainted with the operation of a surgical ward both in the inpatient and outpatient departments. They are to take active part in compiling and keeping health care documentation (clinical notes, etc.). They should learn practically to treat small surgical injuries, to master the dressing techniques and all types of immobilisation of the extremities. They take active part in the work at the operation theatre (assistance at minor surgeries). It is appropriate that the students participate in about two standby duties so that they may learn to know the problems connected with acute surgery. In the course of the summer vacation practical training, students should particularly focus on the following topics: 1. Traumatology cases, especially those concerning the extremities (fractures and luxation), conservative and operative treatment, post-traumatic rehabilitation; 2. Abdominal emergencies, problems of their diagnosis including differential diagnosis and timely operative therapy; 3. Urological emergencies; 4. Pre-operative examination and preparation; 5. Post-operative treatment, intensive care, parenteral nutrition. Student has to pass summer vacation training at one of General Departments of Surgery with chance to attend similar departments (such as Dpt. of Orthopaedics, Neurosurgery, Urology and Plastic Surgery) in the length of 20% (maximum of 2 days) of the length of the summer vacation training from Surgery. The students will keep records on the course of their summer vacation practical training. The head of the surgical institution assesses students both with respect to their theoretical preparedness and as regards their activity in acquiring knowledge and skills.
