Abstract

Případ uvedené pacientky demonstruje inzulinom v jeho běžné podobě. Test s hladověním se i u této nemocné ukázal zlatým standardem v diagnostice inzulinomu. Běžné jsou i potíže se zobrazením ložiska pankreatu, kdy endosonografie je z dostupných metod nejcitlivější. Radiofrekvenční ablace se zároveň ukázala jako velmi vhodná metoda k odstranění p-NET u starší polymorbidní nemocné. Pozitivní somatostatinová scintigrafie nabízela možnost léčby somatostatinovými analogy v případě, že by chirurgická léčba nebyla dostatečně efektivní.

Abstract (in English)

The case demonstrates the insulinoma patient in his normal form. Fasting test as well in this patient showed the gold standard in the diagnosis of insulinoma. Are common and display problems bearing pancreas, when endosonography is the most sensitive methods available. Radiofrequency ablation is also shown as a very suitable method to remove the p-NET polymorbid among older patients. Positive somatostatin scintigraphy offered treatment option somatostatin analogues in the event that surgical treatment was not effective enough.
