Abstract
Obsedantně-kompulzivní porucha (OCD) je závažné duševní onemocnění s častým chronickým průběhem. Za terapii první volby u OCD jsou považována serotonergně působící antidepresiva podávaná ve vysokých terapeutických dávkách po dostatečně dlouhou dobu, anebo kognitivně-behaviorální psychoterapie. Nejzávažnější farmakorezistentní formy OCD mohou kromě ablativního stereotaktického neurochirurgického zákroku těžit z rozvoje nových neuromodulačních technik mozku, jako je repetitivní transkraniální magnetická stimulace (rTMS). Dosud bylo publikováno devět randomizovaně kontrolovaných a tři otevřené studie ověřující účinnost a bezpečnost rTMS při ovlivnění OCD příznaků. Výsledky těchto studií ukazují, že ať již nízkofrekvenční, či vysokofrekvenční rTMS cílená na oblast prefrontálního kortexu není efektivní při léčbě OCD. Naopak oblasti suplementární motorické arey či orbitofrontálního kortexu lze považovat za nadějné pro účinnou a dobře tolerovanou rTMS léčbu u OCD. Budoucí studie by se měly zaměřit na definování optimálních demografických, klinických, stimulačních či neurofyziologických parametrů, které budou spojeny s dobrou terapeutickou odezvou rTMS při léčbě OCD.
Abstract (in English)
Obsessive-compulsive disorder (OCD) is a severe mental disorder with often chronic course. The first option treatment strategies for OCD consist of high doses of serotonin acting antidepressants given over long periods of time and/ or cognitive-behavioral therapy. The most severe resistant forms of OCD may benefit except of ablative stereotactic neurosurgery from development of new types of focal neuromodulatory interventions like repetitive transcranial magnetic stimulation (rTMS). Up to now nine randomized controlled trials and three open trials assessing the safety and efficacy of rTMS in the treatment of OCD were published. The results of these studies show that neither low frequency nor high frequency rTMS focused on the prefrontal cortex is effective in the treatemnt of OCD. By contrast supplementary motor area and/or orbitofrontal cortex seem to be candicate regions for effective and well tolerated rTMS treatment of OCD. Future trials should aim at determining optimal demographic, clinical, stimulation and neurophysiological parameters correlated with good rTMS therapeutical outcome in the treatment of OCD.
