Anotace

On Thursday, 30 May the Surgical Department of the University Hospital Brno held an oncosurgical seminar for students, surgeons, oncologists and gastroenterologists. The theme of this meeting was an interdisciplinary collaboration in the treatment of patients with rectal cancer due to new knowledge and treatment options. The main topic of the seminar was prof. MD. Telekyho from the University of Vienna on "TME in rectal cancer, new trends in the interdisciplinary management". Prof. Teleky is the founder and president of the European Federation for colorectal cancer (EFR), which is engaged in research and postgraduate training in the diagnosis and treatment of colorectal cancer. It is a platform providing support for the development of clinical trials, which is based on cooperation multidisciplinary, international and multifactorial (www.efrcancer.org). The lecture was designed as an overview of the treatment of rectal cancer with highlighting major historical milestones with emphasis on the impact of the current strategy and treatment. He was given an overview of current standard and experimental treatment of previously known results. Finally, the implied trend direction multidisciplinary treatment in our region was indicated. The main benefit of seminar was discussion of current approaches to solving non-standard procedure for patients with complete remission of the cancer after neoadjuvant chemoradiotherapy. Were discussed experiences with different modes preoperative oncological treatment, their benefits, and consequences of the current knowledge on the level of EBM. The communications revealed inconsistencies in procedure due to non-existent long-term results of studies on radical treatment or "watch and wait" strategy in the treatment of so-called complete response of rectal cancer. The discussions provided the current consensus in pursuit of a radical surgical approach with maximum respect conservative approach in the case of a clear desire informed patients requiring three months` oncological dispensary.

Ve čtvrtek 30. května se na Chirurgické klinice FN Brno uskutečnil Onkochirurgický seminář pro studenty, chirurgy, onkology a gastroenterology. Tématem tohoto setkání byla interdisciplinární spolupráce při léčbě pacientů s rakovinou konečníku vzhledem k novým poznatkům a možnostem léčby. Hlavní náplní semináře byla přednáška prof. MUDr. Telekyho z Vídeňské univerzity na téma „TME in rectal cancer, new trends in the interdisciplinary management “. Prof. Teleky je zakladatel a president Evropské Federace pro kolorektální karcinom (EFR), která se zabývá výzkumem a postgraduálním vzděláváním v oblasti diagnostiky a léčby kolorektálního karcinomu. Jde o platformu zajišťující podporu pro vzniku klinických studií, která je založena na spolupráci multidisciplinární, mezinárodní a multifaktoriální (www.efrcancer.org). Přednáška byla koncipována jako přehled vývoje léčby karcinomu rekta s vytýčením zásadních historických milníků s důrazem na dopad na současnou strategii a léčbu. Byl dán přehled současné standardní i experimentální léčby, jejich dosud známé výsledky. Závěrem byly naznačeny tendence směřování multidisciplinární léčby v našem regionu. Hlavním přínosem vlastního sdělení byla diskuse nad současnými možnostmi řešení nestandardního postupu u pacientů s kompletní remisí nádorového onemocnění pro neodajuvatní chemoradioterapii. Byly diskutovány zkušenosti s různými typy režimů předoperační onkologické léčby, jejich výhody, následky a současné poznání na úrovni EBM. Ze sdělení vyplynula nejednotnost postupu vzhledem k neexistujícím dlouhodobým výsledkům studií na téma radikální léčby či „watch and wait“ strategie při léčbě tzv. kompletní remise nádoru rekta. Z diskuse vyplynul současný konsensus ve snaze o chirurgicky radikální přístup s maximálním respektováním konzervativního postupu v případě jednoznačného přání poučeného pacienta s nutností tříměsíční onkologické dispenzarizace.
