Abstract

Chirurgická léčba Crohnovy choroby je indikována při selhání medikamentózní léčby nebo při komplikacích, mezi které patří mj. striktura střeva a ileózní stav. Předkládaná kazuistika popisuje diagnostiku IBD u 17ti letého pacienta s provedenou ileo-transversoanastomózou. Pacient zákrok podstoupil ve věku 14ti let na chirurgickém pracovišti pro dospělé, kdy bylo nutné provedení operační revize dutiny břišní pro ileózní stav. Nynější objektivizace GIT potíží byla při endoskopickém vyšetření ztížena nejasným nálezem v oblasti colon transversum. Po ozřejmění anatomických poměrů bylo provedeno operační zrušení ileo-transversoanastomózy s resekcí preterminálního ilea. Dále byl pacient léčen dle zvyklostí a ve stabilizovaném stavu předán do péče gastroenterologa pro dospělé.

Abstract (in English)

Surgical treatment of Crohn’s disease is indicated by the failure of conservative treatment or by such complications as intestinal stricture and ileus. The present case report describes diagnosis of inflammatory bowel disease in a 17-year-old patient and performance of an ileo-transverse anastomosis. The patient had undergone surgery at age 14 at the adult surgical department, during which time surgical revision of the abdominal cavity had been necessary due to ileus. Objectification of the gastrointestinal tract had been endoscopically aggravated at that time by unclear findings in the transverse colon. After clarification of the anatomical situation, surgical removal of the ileo-transverse anastomosis and resection of the pre terminal ileum had been performed. The patient had then been treated according to guidelines and left in stable clinical condition to the care of an adult gastroenterologist.
