Jedná se o morfologickou odchylku jazyka, na jehož sliznici jsou různě hluboké a v rozličné četnosti přítomné rýhy. Maximum změn je na dorzální straně jazyka nebo na jeho dorzola-terální partii a tak poměrně snadno je lze vidět při obvyklé aspekci dutiny ústní. Histologickým podkladem linguae scrotalis (LS) je výrazné zvětšení hladkých nitkovitých papil (papilae filiformes; ve sliznici jazyka jsou nejpočetnější) s doprovodnou infiltrací intra- a subepiteliální (infiltráty tvoří polymorfonukleáry a lymfocyty). Promiskue je nález označován také: lingua (L) fissurata, L. plicata, L. gyrata, L. cerebriformis.

Lingua scrotalis or fissured tongue (FT) is a condition frequently seen in the adult population. In contrast, in children less than 10 years is FS rare finding. But there is a lack of data on different pattern, severity, and association of fissuring with various systemic disorders and other tongue anomalies. FT is believed to be a normal variant than 10% of the population and perhaps geneticall oriented. On the other hand, local factors implicated in the etiology are ill fitting prosthesis, infection and allergic reaction. Systemic factors concerned with FT are medication, anemia, deficiency of vitamin B complex, zinc, iron, and psychological factors. FT was found to be associated with certain syndromes like Melkersson-Rosenthal syndrome (MRSy), Down's syndrome, and Sjogren syndrome. FT is usually asymptomatic, only few patients complain mild pain. FT does not require any treatment, very important is thorough oral hygiene. Authors describe two patients with FS, one with MRSy and other with FS as a normal variant.
