Abstract
Head and neck cancer accounts for 5 % of malignancies in the regions of Europe and North America. In the course of last thirty years the annual rate of incidence has increased from 6 to 11 newly diagnosed tumors per 100.000 inhabitants. Vast majority of the tumors are not being diagnosed until they reach stage III or IV. Despite significant progress achieved in the field of conservative oncological treatment, surgery still remains prevailing method to treat late stages of head and neck cancer. (1) In our study we present 114 patients, who have undergone extensive surgical removal of tumor followed by the closure of postoperative defects applying microvascular free flap at the ENT clinic of St. Anne’s hospital, Brno between years 2000 and 2014. According to the study, the majority of the patients were first diagnosed with stage III or IV. The study also found out that the highest occurrence rate amongst tumors was recorded in relation to squamous cell carcinoma and basal cell carcinoma. 83 patients were primarily treated with surgery without previous treatment, while 31 persons have first undergone concomitant radio, radio-chemotherapy or primary surgery and undergone salvage surgery as secondary treatment modality. The highest number of reconstruction surgeries were carried out after performing maxillectomies with /or without/ exenterating of orbit. Defect closures were carried out with the application of musculocutaneos flap used from m. latissimus dorsi, m. rectus abdominis and fasciocutaneous forearm Chinese flap, fasciocutaneous lateral arm flap.

Cílem naší práce bylo prezentovat přenosy volných laloků při uzávěrech defektů po onkochirurgických resekcích v oblasti hlavy a krku se zaměřením na chirurgické i onkologické výsledky. Úspěšnost mikrovaskulárních přenosů byla v daném období vysoká - vyšší než 95% a 5-letý DSS téměř 55%.
