Abstract
Cíl výzkumu. Depresivní porucha je velmi rozšířenou duševní poruchou. PHQ-9 (Patient Health Questionnaire-9) je velmi často užívanou metodou pro screening depresivní poruchy pro klinické i výzkumné účely. Cílem studie bylo zhodnotit psychometrické vlastnosti českého překladu metody PHQ-9. Výzkumný soubor. Vzorek studie tvořilo 1654 respondentů (66,6% žen, 33,4% mužů) oslovených prostřednictvím sociálních sítí v rámci širší studie zabývající se motivacemi k sexu. Statistické analýzy. Byla provedena konfirmační faktorová analýza k ověření, která z dříve uváděných faktorových struktur (jedno- a dvoufaktorové řešení) více odpovídá datům. Bylo provedeno testování invariance dvou verzí překladu metody PHQ-9. První verze byla přeložena naším výzkumným týmem, druhá je volně dostupná na stránkách společnosti Pfizer. Dále byla testována invariance pro pohlaví. Výsledky. Výsledky poukazují na dvoufakotorové řešení metody s pěti „somatickými“ položkami a čtyřmi „nesomatickými“ či „emočně-kognitivními“ položkami. Testování invariance překladů metody umožnilo přijmout částečně invariantní model se dvěma položkami lišícími se napříč překlady z hlediska sycení faktoru depresivity. Zhodnocení invariance pro pohlaví v rámci dvou verzí překladu umožňuje přijmout pro verzi DOMOV invariantní model pro muže a ženy a pro verzi Pfizer částečně invariantní model s jednou položkou způsobující neinvarianci. Doporučení z toho plynoucí jsou uvedena v diskuzi. Výsledky položkové analýzy poukazují na vhodnost položek pro škálu, rovněž hodnota vnitřní konzistence metody je uspokojivá. Korelace mezi PHQ-9 a BDI-II je konzistentní s výsledky meta-analytické studie. Metodu PHQ-9 lze považovat za vhodný nástroj pro měření deprese u obecné populace. Limity. Další výzkum by se měl zaměřit na klinickou populaci.
Abstract (in English)
Research objectives. Depression is a very common mental disorder. The Patient Health Questionnaire-9 (PHQ-9) has been frequently used to assess depression in primary healthcare, and in research studies as well. The aim of this study was to assess the Czech version of the PHQ method. Sample and setting. The final study population was 1654 respondents (66,6% of women and 33,4% of men) addressed via social networks within a broader study on motivations for sex. Statistical analysis. We calculated the Confirmatory Factor Analysis to assess which of the previously hypothesized factor structures (a one- or two-factor solutions) would best fit our data. We calculated the Multi Sample Analysis to assess measurement invariance across two versions of method translation into Czech. First version was translated into Czech by our research team. The second version, which is available online, comes from the Pfizer company. We further tested the data for gender invariance. Results. The best fit suggests a two-factor model with five ‘somatic’ items and four ‘non-somatic’ or ‘affective’ items. The model measurement was partially invariant across the two versions of the method's translations. Two items were responsible for non-invariance. The model measurement for version DOMOV was invariant across gender. The model measurement for version Pfizer was partially invariant across gender. One item was responsible for non-invariance. Recommendations are given in the discussion. The item analysis indicates that the items are suitable for the scale and the internal consistency of the scale is satisfactory. The correlation between PHQ-9 and BDI-II is consistent with previous findings of a meta-analytic study. The Czech version of PHQ-9 is a useful tool for assessing depression in the general population. Study limitations. Future research should focus on clinical populations.
