Cíl: Novorozenecká kůže je křehká, vyžadující citlivou péči s minimalizací sekundárního traumatu. V současné době nejsou vždy v léčbě porodních traumat novorozenců využity vhodné terapeutické materiály, ale naopak tradiční postupy, které mohou dítě potenciálně poškodit. Účelem prezentovaného příspěvku je představení možností léčby nehojících se ran u novorozenců s využitím silikonových terapeutických materiálů. Metodika: Prospektivní studie bez kontrolní skupiny (v letech 2008 až 2014). Sledováno a ošetřeno 8 novorozenců s nehojící se ránou vzniklou v důsledku porodního traumatu. K objektivnímu hodnocení stavu dětí byly využity: škála pro hodnocení stavu kůže novorozenců - NeonatalSkin Condition Score (NSCS) a pro hodnocení bolesti Neonatal/lnfant Pain Scate (NIPS). Výsledky/Diskuze: U všech dětí ošetřených silikonovými materiály došlo k výrazné změně v hodnocení pomocí NSCS. V průběhu převazu byly děti klidné s nízkým výskytem pláče a jiných sledovaných bolestivých projevů (změny ve vitálních funkcích - dýchání, tepová frekvence). Závěr: Silikonové terapeutické materiály jsou efektivní v terapii lokálních kožních a slizničních traumat různé etiologie. Umožňují snížení frekvence převazů, vhodný management bolesti novorozenců a minimalizaci sekundární traumatizace křehké kůže novorozenců.

Aim: Newborns' skin is fragile, requiring careful treatment while minimizing secondary trauma. Yet in the treatment of certain post-partum trauma inappropriate materials are used, which can harm the newborn. To evaluate the effect of silicone materials in the treatment of newborns' skin and mucosal traumas. Method: Prospective study with no control group (in the period from 2008 till 2014) at 8 newborns with non-healing wound caused in the early postpartum period (birth related trauma). Objective evaluation of the newborn skin condition (skin integrity) was done with Neonatal Skin Condition Score (NSCS) and pain evaluation with Neonatal/Infant Pain Scale (NIPS). Results/Discussion: For all newborns in those therapeutic silicone materials were used, it has been assessed high efficiency in local changes of their skin according Neonatal Skin Condition Score (NSCS). During dressing changes newborns responded calmly and a lower incidence of crying and other pain symptoms (changes in vital signs - breath, heart rate) was identified. Conclusion: Silicone therapeutic materials are effective in the treatment of local skin and mucosal traumas in newborns of different aetiologies. Those materials allow a reduction in the frequency of dressing changes, appropriate pain management while minimizing the secondary traumatization fragile skin.
