Abstract
Helikobacter pylori je etiologickým faktorem řady onemocnění a jeho eradikace je v indikovaných případech důležitým krokem v jejich terapii. Vzhledem k vyvíjející se rezistenci H. pytori vůči antibiotikům je třeba její průběžná kontrola a úprava terapie na základě výsledků citlivosti H. pylori vůči antibiotikům v dané populaci. Cílem práce je zhodnotit efekt 3. linie eradikační léčby na základě mikrobiologického vyšetření se stanovením citlivosti H. pylori vůči antibiotikům.
Abstract (in English)
Introduction: Helicobacter pylori is the etiological factor of several il lnes ses and its eradication is a key factor in treatment. Due to increas ing resistance to antibiotics, it is neces sary to monitor the development of the H. pylori infection and adjust the course of treatment depend ing on the antibiotic sensitivity of the H. pylori in a given population. The goal of this paper was to evaluate the ef ects of third-line eradication treatment, based on microbiological examinations and as ses sments of H. pylori antibiotic sensitivity. Patients and Methods: The data were col lected on a retrospective and non-selective basis from all patients who had been treated at our department us ing third-line eradication treatment between 2011 and 2015. Antibiotics were selected based on the results of in vitro antibiotic susceptibility tests on biopsy samples of the stomach mucous lining. The ef ect of the treatment was as ses sed by perform ing a biopsy or determin ing the H. pylori antigen in stool samples. Results: Third-line eradication was car ried out on 38 patients. H. pylori cultivation and as ses sment of H. pylori antibiotic susceptibility were succes sful in 31 cases; 23 patients were treated and examined. Eradication was achieved only in three patients, i.e. 7.9% of all cases examined (38). Conclusion: Third-line eradication treatment has so far been unsatisfactory, despite the use of targeted antibiotic treatment, indicat ing that ef orts should be made to increase the ef ectiveness of the frst or second line of eradication treatment.
