Abstract

V kazuistice je popsán případ 49letého pacienta s infiltrací pravé orbity nízce diferencovaným spinocelulárním karcinomem. Pacient podstoupil resekční výkon s následnou adjuvantní léčbou. Došlo nicméně k brzké recidivě a byla indikována další operace, radioterapie a systémová léčba. Další progrese onemocnění však znemožnila jakýkoli kurativní záměr. Protože byla u pacienta imunohistochemicky zjištěna PD-L1 pozitivita v 70 % a možnosti chemoterapie byly vyčerpány, požádali jsme revizního lékaře o schválení úhrady anti-PD-1 protilátky pembrolizumabu. Odpověď na podávanou imunoterapii jsme mohli pozorovat poměrně brzy a byla výrazná, což potvrdilo následné PET/MR přešetření. Prediktivních faktorů pro účinnost imunoterapie je více, avšak ideální stále chybí.

Abstract (in English)

The case report describes the case of a 49-year-old patient with infiltration of the right orbit by low-differentiation squamous cell carcinoma. The patient underwent resection followed by adjuvant treatment. However, there was an early recurrence and further surgery, radiotherapy and systemic treatment were indicated. However, further disease progression precluded any curative intent. Because the patient was immunohistochemically PD-L1 positive in 70% and the chemotherapy options were exhausted, we asked the review physician to approve reimbursement of the anti-PD-1 antibody pembrolizumab. The response to the given immunotherapy could be observed relatively early and was significant, which was confirmed by subsequent PET / MR examination. There are several predictive factors for the effectiveness of immunotherapy, but the ideal is still lacking.
