Abstract

Autoři shrnují nové poznatky v problematice deficitu železa u pacientů se srdečním selháním nedávno reflektované v nových guidelines pro léčbu srdečního selhání. Vedle rekapitulace dřívější evidence se věnují výsledkům studie AFFIRM-HF, které rozšířily indikaci karboxymaltózy železa i pro nemocné po akutní dekompenzaci srdečního selhání. Bylo prokázáno, že po intravenózní suplementaci železa dochází ke zlepšení symptomů, výkonnosti a snížení počtu hospitalizací pro dekompenzaci srdečního selhání. Text je doplněn kazuistikou popisující případ 60leté pacientky se srdečním selháním ischemické etiologie, u které byl při zhoršení stavu zjištěn deficit železa a po intravenózním podání karboxymaltózy železa došlo k výraznému zlepšení symptomatologie. Kazuistika je doplněna přehledem vlastních zkušeností při sledování a léčbě deficience železa u pacientů se srdečním selháním.

Abstract (in English)

The authors summarize the new findings on iron deficiency in patients with heart failure recently reflected in the new guidelines for the management of the heart failure. In addition to recapitulating previous evidence, they discuss the results of the AFFIRM-HE trial, which extended the indication of ferric carboxymaltose to patients after acute decompensation of heart failure. It has been shown that after intravenous iron supplementation there is an improvement in symptoms, performance and a reduction in hospital admissions for heart failure decompensation. The text is supplemented by a case report describing the case of a 60-year-old female patient with heart failure of ischemic etiology who was found to be iron deficient when her condition worsened, and there was a significant improvement in symptomatology after intravenous administration of ferric carboxymaltose. The case report is followed by a review of our own experience in monitoring and treating iron deficiency in patients with heart failure.
