The purpose of the study has been to evaluate the influence of one year angiotensin I-converting enzyme inhibitor (IACE) treatment on 24-h blood pressure (BP) and heart rate (HR) parameters, other hemodynamic (ECG, ECHO) parameters and hematological and biochemical tests in the genetic context of renin - angiotensin system gene polymorphisms. Eleven newly diagnosed essential hypertensives were treated with ACE inhibitor quinapril (Accupro, Gödecke, Berlin) for one year. All patients were genotyped for three polymorphisms - I/D of angiotensin I-converting enzyme and M235->T and T174->M of angiotensinogen genes, respectively. A decrease in the mean levels of systolic (p=0.001), mean arterial (p=0.002) and diastolic (p=0.003) blood pressures were found. No significant differences in ECG parameters nor differences in ECHO parameters became evident.However, a decrease in the red blood cell count (p=0.004) and an increase in the MCV of the RBCs (p=0.002) were observed. Differences in the blood glucose levels appeared to be associated with I/D ACE genotype (p=0.01).
