Mezi nejčastěji uváděné rizikové faktory vzniku OSA u dětí se řadí obezita, zvětšené krční a nosní mandle, neuromuskulární onemocnění, a v neposlední radě se významně na jejím vzniku podílí kraniofaciální malformace. Kostěné části kraniofaciálního systému tvoří oporu horních dýchacích cest (HDC) a jejich důležitost pro fyziologické dýchání jedince se projevuje zejména ve spánku, kdy dochází ke změnám v kontrole svalového tonu a reflexním odpovědím. Kraniofaciální anomálie mohou přispívat k zúžení, snadnějšímu kolapsu a okluzi dýchací cest. Jelikož jsou ortodontisti specialisty, kteří se zabývají problematikou orofaciální oblasti a anomáliemi obličejového skeletu u rostoucích dětských pacientů, mohou zásadním způsobem přispět jak k včasné diagnostice, tak i k neinvazivní léčbě pacientů s dětskou OSA. Metodika: V rámci vstupního ortodontického vyšetření se standardně zhotovuje kefalometrický snímek, telerentgen, neboli snímek lebky pacienta z bočního pohledu. Na základě jeho analýzy ortodontista určuje vzájemný vztah čelistí a popisuje skeletální podklad dané ortodontické anomálie. Tento snímek zároveň dvojrozměrně zobrazuje HDC a umožnuje tak jejich základní kontrolu, a to i ve vztahu k čelistem. Výsledky: Kefalometrické studie s dětskými pacienty trpícími OSA poukazují na charakteristické znaky, které mohou ovlivňovat průchodnost dýchacích cest a podílet se tak na vzniku OSA. Časté je zúžení nasomaxilárního komplexu, viditelně úzké a klenuté tvrdé patro, mnohdy vedoucí k vzniku jedno- nebo oboustranně zkříženého skusu. U pacientů se vyskytuje retrognátní typ obličejového skeletu, hypoplasie střední obličejové etáže, mikrognácie nebo mikrogenie, tedy podvývoj čelistí. Rizikové je i zvýšení divergence čelistních bází, tzv. skeletálně otevřený skus, projevující se i nedostatečným retním uzávěrem. Správně načasovaná ortodontická léčba u dětí s dysmorfizmem, vedoucím k zúženým HDC, může stimulovat růst či změnu polohy čelistí, zlepšit tak jejich správný vývoj, a zároveň tak snížit riziko jejich kolapsu. Příkladem je v rámci ortodontické terapie poměrně často používaná maxilární expanze, případně rychlá maxilární expanze (RME). Při RME dochází k expanzi maxily a rozšíření zubního oblouku, zároveň se však expanduje i nosní spodina a zvětšuje se objem nasofaryngu, intranasální kapacita roste, čímž dochází ke snížení nosní rezistence. U pacientů trpících maxilární konstrikcí a příznaky OSA tak RME prokazatelně zlepšuje nosní ventilaci a nabízí se jako vhodná terapie. Závěr: Ortodontická léčba se jeví jako úspěšný terapeutický přístup OSA u dětských pacientů s anomáliemi orofaciálního skeletu, přičemž klíčovým aspektem při terapii tohoto onemocnění stále zůstává podrobná anamnéza, včasná diagnóza a stanovení příčinných faktorů s následným interdisciplinárním přístupem při terapii.
