Reporting adverse events is important for increasing the quality of care for patients. This is an international problem in all medical facilities that provide patient care. Many experts and researchers worldwide are dealing with this topic to find the best way not only for adverse events monitoring but especially for preventing harm to patients and promoting safety guidance and pathways used in clinical practice.
