Abstract

Optimální terapeutický postup v léčbě nemalobuněčného plicního karcinomu nebyl doposud nalezen. Systematická mediastinální lymfadenektomie, která je metodou volby při chirurgické resekci plicního nádoru, zvyšuje perioperační riziko a její význam není zcela vyjasněn. Metoda detekce a biopsie sentinelové uzliny nabízí slibnou alternativu ke standardnímu postupu. Dosavadní výsledky je třeba potvrdit většími studiemi. Indikace k použití metody budou pravděpodobně omezeny na stadium Ia nemalobuněčné plicní rakoviny. Nezbytnou se v příštích studiích jeví kombinace scintigrafie a barvení lymfatik patentní modří. V přehledu je shrnut současný stav problematiky a navržen možný postup na základě vlastních zkušeností

Abstract (in English)

The optimal type of treatment of the non-small cell lung cancer has not been found yet. Systematical mediastinal lymphadenectomy, which is the method of choice in the surgical resection of the lung cancer, is potentially risky and its role is still unclear. The sentinel lymph node technique represents a promising alternative to the standard treatment. The hitherto reached results have to be evaluated with care and greater studdies are necessary. According to our opinion, the best results can be expected in stage Ia NSCLC. The combination of the intraoperative scintigraphy and blue dye technique will be probably necessary in the next studies. The review gives the current state-of-art and some suggestions to the future.
