Abstract
Jako mnoho komplexních biologických fenoménů, začíná být také bolest studována na úrovni genů. Vývoj molekulární biologie v posledních letech umožnil nejen klonování, mapování a sekvenování genů, ale také jejich ovlivování prostřednictvím technik knock out, oligo antisense nebo virových vektorů. Tyto metody umožňují zkoumat na molekulární úrovni, které z cca 30 a 40 tisíc genů se podílejí na zprostředkování bolesti, které z nich jsou polymorfní a které polymorfizmy jsou zodpovědné za interindividuální rozdíly ve vnímání bolesti. Velkým problémem při studiu bolesti je její emoční složka. K dalším problémům patří skutečnost, že nástroje, které by byly použitelné pro měření bolesti a její srovnávání mezi jednotlivými osobami, jsou nedostatečné. Přes tyto problémy již dnes máme k dispozici mnohé informace, které by bez metod molekulární biologie a bez experimentů na zvířatech byly jen těžce přístupné. Teprve nedávno se podařilo prokázat genetický původ familiární hemiplegické migrény a kongenitální insenzitivity bolesti s anhidrózou. V posledních šesti letech je genetický výzkum bolesti zacílen především na možnost genové terapie pacientů s chronickou bolestí. Výsledky těchto studií jsou povzbudivé a lze říci, že již v blízké budoucnosti budou použitelné v klinické praxi.
Abstract (in English)
As many complex biological phenomena, pain starts to be studied on the genetic level. The development of molecular biology in the last years enabled not only cloning, mapping and sequencing of genes but also a research of genes by the techniques of knock out, oligo antisense or viral vectors. These methods help us to find, which of 30 40 thousands genes influence the pain feeling, which of genes are polymorphic and which of the polymorphisms are responsible for interindividual differences in pain feeling. The emotional part of pain represents a big problem in the pain research. Moreover, the tools that could be used for the pain measurements and for the pain comparison between the individuals are insufficient. In spite of these problems, a lot of information is available thanks to molecular biology and the experiments on animals. Recently, the genetic basis of familial hemiplegic migraine and congenital insensitivity to pain with anhidrosis were proved. During the last six years, genetic research of pain is focused mostly on the possibility of gene therapy of patients with chronic pain. The results of these studies are encouraging and one can say that in next future they could be used in clinical practice.
