Objective: Enhanced oxidative stress following tracheobronchial surgery is presumed to have a profound impact on the patients postoperative course. A large number of factors are involved in the process of maintaining the pro-/antioxidative balance in humans with genetic variability in antioxidant enzymes being one of them. The aim of the study was to investigate relationship between selected substitutions in genes encoding for antioxidant enzymes (GPx, MnSOD, CAT) and severity of acute tracheobronchitis following tracheotomy. Materials & Methods: Prospective study of 44 patients after tracheotomy. Indications for tracheotomy were: scheduled surgery for the diagnosis of HNSCC, urgent tracheotomy due to the respiratory insufficiency following the obstruction of upper airways by HNSCC (n=40) or obstruction of an upper respiratory tract caused by other reasons than tumour. Parameters describing the postoperative course plus results of an endoscopic examination of inflammatory changes in tracheobronchial tree were used for calculation of the tracheobronchitis score identified as KTBscore. Endoscopic evaluation of tracheobronchitis severity was scored as the Endoscore. Genotypes of the GPx1 Pro197Leu, MnSOD Ala16Val and CAT -262C/T polymorphisms were detected by means of PCR-based methodology. Results: Allele frequencies were 62.5% Pro, 37.5% Leu of the GPx1 Pro197Leu; 42.9% Ala, 57.1% Val of the MnSOD Ala16Val and 74.3% C allele, 25.7% T allele of the CAT -262C/T. Genotype frequencies of all three polymorphisms studied fulfilled criteria of Hardy-Weinberg equilibrium (P=0.05, chi2 test). Comparison of mean KTBscore, Endoscore and type of cultivated bacteria among particular genotype groups revealed no significant differences (P=0.05, Kruskal-Wallis ANOVA). Conclusions: Based on these pilot results, there is no indication that polymorphism in selected genes encoding for antioxidant enzymes influence both the postoperative course and development of tracheobronchitis following tracheotomy. However, number of subjects enrolled in the study so far represents only a fraction of the planned total number of probands (approx. 240). Therefore, any conclusions drawn so far have to be considered preliminary and speculative. Cílem práce bylo stanovit vztah mezi vybranými variantami genetických polymorfizmů pro antioxidativní enzymy (GPx, MnSOD, CAT)a závažností průběhu akutní tracheobronchitidy po tracheotomii. Do studie jsme zařadili 44 pacientů po tarcheotomii z následujících indikací: v rámci plánovaného onkochirurgického výkonu pro spinocelulární karcinomy hlavy a krku, akutní zajištění dýchacích cest u pacientů s karcinomy hlavy a krku a obstrukce dýchacích cest u pacientů bez nádorů. na základě těchto pilotních výsledků zatím nemáme indicie pro výrazný vliv genetických polymorfizmů pro antioxidativní enzymy na pooperační průběh po tracheotomii. Vzhledem k nízkému počtu pacientů ve studii je nutné i tyto výslekdy pokládat za předběžné. Prace byla podpořena grantem IGA NR/7996-3.
