Osteonekróza čelisti v souvislosti s léčbou bisfosfonáty je novým fenoménem. Popisujeme případ muže ve věku 41 let, u něhož se osteonekróza čelisti vyvinula v průběhu protimyelomové léčby. Pacient měl problémy se zubem dolní čelisti a od počátku chemoterapie a podávání bisfosfonátů. K extrakci zubu bylo přistoupeno ale až měsíc před podáním vysokodávkované chemoterapie s autologní transplantací krvetvorných buněk, neboť dosavadní konzervativní léčba zubu nebyla úspěšná. Pacient od počátku léčby dostával perorální formu klodronátu, později však došlo k výměně bisfosfonátu za zoledronát. Po roce od extrakce zubu, když při konzervativním ošetřování nedošlo ke zhojení rány po extrahovaném zubu, byla proveden resekce nekrotické kosti. Díky operaci se výrazně zmenšila bolestivost, ale nedošlo k úplnému zhojení rány. Po stanovení diagnózy osteonekróza čelistí bylo podávání bisfosfonátů přerušeno.

The osteonecrosis of jaws is a phenomenon newly described in the connection with bisphosphonate therapy. We describe a case of a 41-year-old man with multiple myeloma who developed an osteonecrosis of the jaw in the course of anti-myeloma therapy. The man had been suffering from toothache since the beginning of the chemotherapy, nevertheless the extraction was not performed until one month before high dose chemotherapy with peripheral stem cell transplantation started by reason that the conservative therapy had proved unsatisfactory. Bisphosphonates were administered from the beginning of the chemotherapy; the bisphosphonate therapy was started with clodronate, which was later replaced by zoledronate. One year after performing the tooth extraction the wound stil would not heal up therefore resection of the necrotic bone was performed. The pain of the jaw diminished but the wound did not heal up completely. The administration of bisphosphonates was stopped after setting the diagnosis of osteonecrosis of the jaw.
