Abstract

Nejprve jsou shrnuty obecné zásady bipolární poruchy. Dále se sdělení zabývá léčbou bipolární deprese. Do popředí zájmu se nyní dostávají v této indikaci antipsychotika 2. generace. Nedávno byly publikovány dvě dvojitě slepé kontrolované studie u bipolární deprese s olanzapinem a quetiapinem. Detailně jsou uváděny výsledky dvojitě slepé, randomizované, placebem kontrolované studie s quetiapinem. Monoterapie quetiapinem v dávkách 300 a 600 mg denně vedla ke statisticky signifikantně většímu zlepšení než placebo ve všech použitých škálách, hodnotících ovlivnění depresivní a úzkostné symptomatologie, spánku a kvalitu života již od prvního týdne, a byla dobře tolerována.

Abstract (in English)

At first general principles of treatment of bipolar disorder are summarized. Further the paper is dealing with the treatment of bipolar depression. In this indication the treatment with atypical antipsychotics is becoming a point of interest. Recently, two double-blind, randomized placebo-controlled studies in patients with bipolar depression comparing olanzapine and combination of olanzapine and fluoxetine with placebo and quetiapine with placebo were published. Quetiapine monotherapy at doses of 300 and 600 mg pro die demonstrated stapared with placebo from week 1 onward and was well tolerated.
