Abstract

Normotenzní hydrocefalus je onemocnění, které se projevuje typickou klinickou triádou : poruchami chůze, močovou inkontinencí a poruchou kognitivních funkcí, doprovázené nálezem ventrikulomegalie na zobrazovacích metodách a normálním nitrolebečním tlakem. V diagnostice a ke zjištění tzv. shunt-responsive pacientů u normotenzního hydrocefalu kromě anamnézy, podrobného klinického zhodnocení a vyšetření zobrazovacími metodami užíváme nejčastěji lumbální infuzní test se stanovením tzv. výtokového odporu (Rout) a kontinuální lumbální drenáž. Na našem souboru pacientů za posledních 5 let autoři prezentují svoje zkušenosti s diagnostikou a léčbou nemocných s normotenzním hydrocefalem. Jedinou efektivní léčbou normotenzního hydrocefalu je implantace ventrikulo-peritoneálního, event. ventrikuloatriálního shuntu. Důležitou součástí celého procesu je řádná diagnostika onemocnění a stanovení tzv. shunt-responsive pacientů.

Abstract (in English)

Symptomps of normal pressure hydrocephalus are usually walking problems, urinary symptoms and dementia. Ventriculomegaly is presented on imaging exams and intracranial pressure is at normal range. Suplemental tests (lumbar infusion test and external lumbar drainge) are used to find "shunt-responsive" patients. Authors present their own experiences with diagnosis and treatment of normal pressure hydrocefalus during last 5 years. The implantation of ventriculoperitoneal or ventriculoatrial shunts is standard treatment of normal pressure hydrocephalus.
