Do popředí zájmu se dostávají tzv. duální antidepresiva typu SNRI (specifické inhibitory zpětného vychytávání serotoninu a noradrenalinu). Nejdéle je k dispozici venlafaxin, o kterém byla publikována řada přehledů i v české odborné literatuře. V této práci jsou shrnuta novější srovnání venlafaxinu s ostatními antidepresivy, která upřesňují jeho postavení. Ukazuje se, že antidepresiva ovlivňující dostupnost více než jednoho ze základních neurotransmiterů mají razantnější účinek. Venlafaxin je dnes v ČR dostupný ve formě s prodlouženým uvolňováním, která je lépe snášena. Perspektivní je zavedení desvenlafaxinu, aktivního metabolitu venlafaxinu, do klinické praxe. K jeho hlavním výhodám patří, že není metabolizován enzymatickým systémem CYP2D6 a tento systém - na rozdíl od venlafaxinu - neinhibuje. V praxi to může znamenat menší riziko lékových interakcí.

Dual-action antidepressants SNRIs have recently come into the limelight. Venlafaxine, the longest available SNRI, has also been the subject of a number of review articles in the Czech literature. This article summarizes the most recent comparisons of venlafaxine with other antidepressants, thus defining the place of the drug a mong other antidepressants. Antidepressants that influence the availability of more than one of the major neurotransmitters appear to have a more pronounced effect. In the Czech Republic, venlafaxine is currently available in the better tolerated extended-release form. Desvenlafaxine, an active metabolite of venlafaxine, has shown great promise in clinical practice. One of the greatest advantages of desvenlafaxine is that it is not metabolized via CYP2D6 and, unlike venlafaxine, does not inhibit CYP2D6. This fact would have the practical benefit of reducing the risk of drug interactions.
