Abstract |

Roztroušená skleróza je chronické autoimunitní onemocnění charakterizované mnohočetnými zánětlivými ložisky v bílé hmotě mozku a míchy. V patogenezi tohoto onemocnění hraje důležitou roli dysregulace imunitního systému, která umožňuje autoagresivním T lymfocytům reagovat s antigeny centrálního nervového systému. Za zajištění autotolerance jsou zodpovědné regulační T lymfocyty (Th3 buňky, Treg buňky), jejichž deficit byl popsán u řady autoimunitních onemocnění. Cílem naší studie je zjistit zastoupení jednotlivých subpopulací T lymfocytů a jejich ovlivnění léčbou u pacientů s roztroušenou sklerózou ve srovnání se zdravými jedinci. Metodika Do studie jsou zařazováni pacienti s remitentně relabujícím průběhem roztroušené sklerózy s EDSS méně než 4,5, kteří začínají s imunomodulační léčbou. Vylučovacím kritériem pro zařazení do studie je předchozí léčba kortikoidy či jinými imunomodulačními preparáty v posledních 30 dnech a zánětlivé a nádorové onemocnění. Pomocí průtokového cytometru jsou stanoveny počty následujících subpopulací lymfocytů- CD3+, CD3+CD4+, CD3+CD8+, CD3+CD4+CD45RA+, CD3+CD4+CD45RO+, CD3+CD4+CD45RO+CCR7+, CD3+CD4+CD45RO+CCR7-, CD3+CD8+CD28-, CD3+CD4+CD25+, CD3+CD4+CD25+CD127-. Vzorky periferní krve jsou analyzovány před léčbou a následně po 1, 3 a 6 měsících léčby. Jako kontrolní skupiny jsou použity vzorky krve zdravých dárců. Výsledky a závěr Předběžná data jsou získány analýzou 60 pacientů s remitentně relabující roztroušenou sklerózou s průměrným věkem 33,58 + 7,36, EDSS 2,23 + 0,77. Z této skupiny je 21 pacientů léčených interferonem beta 1-a, 11 interferonem beta 1-b, 12 glatirameracetátem, 5 kortikoidy, 11 neléčených. V souboru zaznamenáváme významné snížení počtu imunoregulačních CD3+CD8+CD28- lymfocytů u pacientů ve srovnání se zdravými kontrolami. Dále se ukazuje trend k vzestupu imunoregulačních T lymfocytů s léčbou imunomodulačními preparáty. |

Background: Multiple sclerosis (MS) is a chronic disease characterized by demyelination and chronic inflamation of the central nervous system (CNS). Many of the immune cells including T cells, B cells seem to be involved in disease pathogenesis by inducing or controlling the immune responses in the nervous system of MS patients. The aim of our research was to study subpopulations of T cells in newly diagnosed MS patients. Material and methods: Fifty eight patients with relapsing-remitting MS (RRMS; age 17 - 57 years, median 32 years) were included in the study. These patients were newly diagnosed of RRMS. Expanded disability status scale (EDSS) was recorded in all patients at the Department of Neurology, University Hospital Brno. Patients were excluded from the study if they had previously received IFN beta 1a, IFN beta 1b or glatiramer acetate. None of the patients received any immunomodulatory or immunosuppressive drugs at least 30 days before blood sample was taken for the purpose of the study. A control group consisted of 40 age-matched healthy volunteers (age 18 – 62, median age 28). Using four-color flow cytometry we identified naive T-cells CD4+CD45RA+, central memory T-cells CD4+CD45RO+CCR7+, effector memory T-cells CD4+CD45RO+CCR7-, T-regulatory cells (Treg) CD4+CD25highCD127low and T-suppressor cells CD8+CD28-. These data were analysed with non-parametric Mann–Whitney test, Kruskal–Wallis test, Spearman’s correlation analysis. Results: Increased numbers in populations of central memory T-cells (p = 0.014) and effector memory T-cells (p > 0.001), decrease of naive T-cells (p = 0.017) and no difference in Treg cells (p = 0.882) and T-suppressor cells (p = 0.162) were observed between MS patients and healthy controls. In order to detect any correlation between severity of MS clinical symptoms and immunological changes we correlated studied T-cell populations with EDSS score. We found increasing numbers of central memory T-cells (p = 0.014) and effector memory T-cells (p = 0.007) with increased of EDSS. No correlation between naive T-cells, Treg cells and Ts cells and EDSS (p = 0.195, p = 0.318, p = 0.683, respectively) was noticed. Conclusions: Our results showed imbalance of immune system of MS patients compared to healthy controls. |
