Abstract

Interdisciplinární paliativní tým patří mezi základní a klíčové metody funkčního paliativního programu.V ideálním případě jednotlivé profese navzájem využívají své vědomosti, dovednosti a zkušenosti při poskytování holistické péče. Funkční tým však není samozřejmostí a na spolupráci jednotlivých profesí je třeba pracovat. Podmínkou vzniku dobrého týmu jsou správní lidé a vhodný vedoucí. V týmu je třeba si vyjasnit cíle a očekávání jednotlivých členů týmu. Důležitá je komunikace v týmu a pečlivý výběr případných nových členů týmu. V týmu by měli být zatoupeni jak pracovníci z praxe, tak pracovníci zabývající se problematikou v teoretické rovině. Tým by se měl scházet pravidelně a pokud možno vždy před návštěvou pacienta i po ukončení této návštěvy.

Abstract (in English)

Interdisciplinary palliative team is one of the basic methods and key functional palliative programu.V ideally each other professions use their knowledge, skills and experience in providing holistic care. The team is not a matter of course and the different professions, it should be working. Prerequisite for the good team are the right people and the appropriate supervisor. The team must be clear objectives and expectations of individual team members. The communication in the team and careful selection of any new team members. The team should be zatoupeni as workers from the practice and staff to deal with the problem in theory. The team should meet regularly and, if possible, before visiting the patient after the end of this visit.
