Aerococcus urinae is a rarely reported pathogen usually causing mild urinary tract infections, but serious complications such as endocarditis and septicemia have also been described. It might be easily missed or misidentified using commercial detection systems. To our knowledge, fourteen patients of A. urinae infective endocarditis, including 8 fatal cases, have been reported in the literature so far. Here we describe a new case of blood culture negative infective endocarditis in 69-year-old male. The patient recovered fully after treatment by aortic valve replacement and antibiotics. The causative agent was identified from the valve by broad-range PCR.
