Abstract

Souhrn: Autor se zabývá problémy diagnostiky a léčby cervikální myelopatie v širším slova smyslu. Upozorňuje především na rozpoznání nejčastější krční myelopatie, kterou je kompresní spondylogenní myelopatie, a její diferenciální diagnostiku, na nebezpečí jejího pod- a přediagnostikování. Další velkou skupinu tvoří míšní tumory, jejichž diagnostika a především léčba má řadu úskalí. Časté diagnostické rozpaky přináší amyotrofická laterální skleróza, která může být snadno zvl. v počátečních stadiích zaměněna za spondylogenní cervikální myelopatii pro ubikvitárnost degenerativních změn na páteři; pacienti pak bývají zbytečně operováni. Mnoho problémů je spojeno rovněž s akutní transverzní myelitidou, protože její příčina je mnohoznačná a její rozpoznání svízelné.

Abstract (in English)

Abstract: This article deals with the difficulties involved in the diagnosis and differential diagnosis of cervical myelopathy. The author places particular emphasis on the identification of compressive spondylotic myelopathy and its differential diagnosis and points out the risks of under- and over-diagnosis. A further large group of cervical myelopathies is represented by spinal tumours, the diagnosis and treatment of which are associated with many pitfalls. Amyotrophic lateral sclerosis is also associated with diagnostic problems because it may easily be mistaken for spondylotic cervical myelopathy in terms of the ubiquity of degenerative changes of the spine, leading to unnecessary operations. Many problems are related to acute transverse myelitis, because this term has often been used interchangeably in the published literature; the aetiology is ambiguous and recognition therefore tricky.
