Abstract

The aim was to evaluate the etiology in a group of 84 patients with painful sensory neuropathy with predominant small-fiber dysfunction(54 men,30 women,median:58; range:25-83 years).Involvement of small nerve fibers was verified by abnormal thermal thresholds and/or reduced intraepidermal nerve fiber densities.Motor signs or symptoms or significant clinical signs of sensory large-fiber involvement were exclusionary;33 patients,however,had sensory nerve conduction abnormalities.The multivariate regression model disclosed that diabetes mellitus(odds ratio [OR]=4.08),chronic alcoholism(OR=5.31),and serum cholesterol levels(OR=4.51)were the only parameters independently associated with small-fiber involvement.No possible etiology was detected in 19 patients(22.6%).In conclusion,the spectrum of risk factors and proportion of idiopathic cases in geographically defined small-fiber polyneuropathy sample is similar to that referred in large-fiber polyneuropathy.

Abstract (in Czech)

Cílem studie bylo zhodnotit etiologii ve skupině 84 nemocných s bolestivou senzitivní neuropatií a predominantním postižením tenkých vláken (54 mužů a 30 žen, věk 58; 25-83 let [median; rozptyl]) pocházejících z populace jihomoravského regionu České republiky. Postižení tenkých vláken bylo ověřeno abnormálním termickým prahem a/nebo sníženou hustotou intraepidermálních nervových vláken. Motorické příznaky nebo významné příznaky postižení silných senzitivních vláken byly vylučujícím kritériem; 33 nemocných však mělo abnormality senzitivní neurografie. Pro srovnání byla prevalence rizikových neuropatických faktorů zhodnocena rovněž ve skupině 47 asymptomatických kontrol párovaných s ohledem na věk a pohlaví (30 mužů a 17 žen, věk 59; 29_85 let). Model vícerozměrné regresní analýzy odhalil, že diabetes mellitus (relativní riziko [RR] = 4,08), chronický alkoholismus (RR = 5,31), a sérový cholesterol (RR = 4,51) byly jedinými parametry nezávisle asociovány s postižením tenkých vláken. Žádná potenciální příčina nebyla zjištěna u 19 patientů (22,6%). Závěrem: spektrum rizikových faktorů a podíl idiopatických případů v geograficky definovaném vzorku nemocných s neuropatií tenkých vláken jsou podobné jako údaje uváděné v literatuře u neuropatie silných vláken.
