Abstract
Autor se zabývá výsledky, srovnává výhody a nevýhody jednotlivých metod a poukazuje na to, že v současnosti neexistuje žádný důkaz podporující nějakou výhodu roboticky asistované radikální prostatektomie, pokud jde o invazivitu, zachování kontinence moči a erekce. Hodnotí i onkologické výsledky méně invazivních metod, které jsou spojeny s častějším výskytem tzv pozitivních chirurgických okrajů. Průzkumy dále ukazují, 4,4x vyšší míru spokojenosti u pacientů podstupujících otevřenou operaci, než u pacientů léčených roboticky asistovanou metodou Adjuvantní terapie byla častěji (3x) zapotřebí u pacientů podstupujících robotickou operaci, než ve druhé skupině.
Abstract (in English)
The author discusses outcomes and drawbacks of individua techniques and shows that there is currently no evidence supporting any benefit of robotic-assisted ra dical prostatectomy in terms of invasiveness, recovery and maintaining of urinary continence and erection. The results of these less invasive techniques in terms of oncologic outcome ere even worse – they are associated with higher incidence of so-called positive surgical margins. Surveys show 4,4times higher satisfaction rate in patients undergoing open surgery than in patients treated with robotic-assisted surgery. The adjuvant therapy was more (3times) frequently required in patients undergoing robotic-assisted surgery than in the other group.
