Unfortunately, more than 50% of psychiatric patients are non-compliant. Clinical pharmacists can be involved in the improvement of patients’ adherence. This study was performed with the aim to introduce a new tool to improve the adherence of psychiatric patients during hospitaliza- tion. This tool has been developed by a clinical pharmacist and it is based on the combination of a com- prehensive medication review and patient education interview. Additionally, patients are provided with educational materials. Different drug-related problems were identified using this new tool (comprehensive medication review and patient education interview). There were suggested 1,136 drug-related pharmaceu- tical interventions in the psychiatric hospital during the 1-year period for hospitalized patients (educated and non-educated). The majority (84%) of these interventions were fully accepted by physicians, and it shows a high clinical relevance. The variety of adherence barriers was defined in a group of educated patients and possible interventions to overcome them were suggested as the outcome of this study. The combination of a comprehensive medication review and education interview might be an effective tool to increase the level of patients‘ adherence. It shows that patients’ responsibility and personal motivation are strengthened by the education interview with clinical pharmacists. This method helps to identify dif- ferent barriers to adherence, possible risk factors, and drug-related problems, which belong to the leading mechanisms for low patient adherence. ER -
