After the first psychotic episode the course of disease it highly variabe. The acute treatment response may not be the decisive factor. It is necessary to look after prognostic factors identifying patients with worse outcome already during the first psychotic manifestation. Further factors that may underlie the acquisition of insight should be studied, because insight has implications for treatment adherence.

Vývoj onemocnění po první epizodě schizofrenie je velmi různorodý. Akutní příznaky onemocnění jsou rozhodujícím faktorem. Nezbytný poznávací prognostický faktor pro identifikaci stavu pacienta je horší výsledek po roce sledování než během prvních psychotických projevů. Dalším důležitým faktorem, při jehož zachování by se měly projevit výsledky léčby u sledovaných pacientů, by mělo být důkladné proniknutí do problematiky onemocnění.
