Abstract

Duálně působící antidepresiva jsou relativně novou skupinou. Hlavními zástupci jsou inhibitoři zpětného vychytávání serotoninu a noradrenalinu (SNRI), mezi které patří venlafaxin, milnacipran a duloxetin. Sdělení se zabývá milnacipranem. Detailněji je rozebírána nová indikace fibromyalgie. V současné době je zdůrazňována individualizovaná léčba. Určitě drobné rozdíly ve farmakokinetice a farmakodynamice SNRI mohou být důležité při individualizované léčbě. Jsou uvedeny kazuistiky nemocných úspěšně léčených milnacipranem.

Abstract (in English)

Dual - acting antidepressants are relatively recent class of antidepressants. They are represented maintly by resotonin and norepinephrine reuptake inhibitors (SNRI), e. g. venlafaxine, milnacipran and duloxetine. The article deals with milnacipran. The new indication, fibromyalgia, is discussed in more details. Nowadays, the individualized treatment is emphasized. Certain differences in the pharamacodynamics and pharmacokinetics of SNRI may be important to tailor the treatment choice to the patient. Some case reports of patients successfully treated with milnacipran are described.
