Abstract

In this trial, we have evaluated the potential of combinative regimen CVD (Cyclophosphamide, Bortezomib (Velcade), Dexamethasone) reduced intensity (CVD senior; 50% reduction) in elderly pts and/or pts with poor status performance with relapsed MM. The results were compared with CVD junior regimen outcome used at the same time in pts aged < 65 years and good status performance. The reduced CVD senior regimen is tolerated better than CVD junior. Duration of treatment and number of cycles was similar in both regimens. There was no difference in overall response but the higher total dose of bortezomib achieved by pts in each group resulted in a better response in both groups. The reduced intensity CVD regimen seems to be a good option for elderly pts with poor status performance with well balanced efficacy/toxicity ratio.

Abstract (in Czech)

V klinické studii byl hodnocen potenciál léčebného režimu CVD (cyklofosfamid, bortezomib (Velcade), dexametazon) v redukované intenzitě (CVD senior, redukce o 50%) u pacientů s relabovaným mnohočetným myelomem, kteří jsou starší 65 let a/nebo jsou v horším klinickém stavu. Výsledky byly porovnány s výsledky pacientů mladších 65let a v dobrém klinickém stavu, kteří byly léčeni režimem CVD junior (v plných dávkách). Redukovaný režim CDV senior byl tolerován lépe jak CVD junior. Trvání léčby a počet podaných cyklů léčby byl podobný u obou režimů. Nebyl zaznamenán rozdíl v celkové léčebné odpovědi mezi oběma režimy, ale podání vyšší dávky bortezomibu bylo spojeno s lepší léčebnou odpovědí nezávisle na podaném režimu. Režim CVD senior s redukovanou intenzitou se zdá být dobou léčebnou možností pro starší pacienty v horším klinickém stavu s dobrým poměrem účinnost/toxicita.
