Abstract

Většina lidí v průběhu svého života potřebuje stomatologické ošetření. U nemocných do té doby zdánlivě zdravých to může být situace, při níž dojde ke zjištění vrozeného krvácivého onemocnění. U nemocných s již známou krvácivou diatézou to je situace, která vyžaduje dobrou interdisciplinární spolupráci. Nejčastěji jsou mez pacienty s poruchou krevního srážení ošetřováni nemocní s antikoagulační léčbou. Naprostá většina z nich však nevyžaduje přerušení léčby, jen znalost aktuální terapeutické hladiny INR a náležitou odbornou erudici stomatologa. Jiná situace je u nemocných se závažnou poruchou krevního srážení. Tito vyžadují nejen speciální stomatologické ošetření u zubních extrakcí, ale i celoživotní stomatologickou péči. Lze určit i nemocné, u nichž není možné pouhé ambulantní ošetření, ale je nutná i několikadenní hospitalizace, a to i v případě jednoduchýách extrakcí zubů. Mezi ně patří zejména nemocní s hemofilií a inhibitorem, pacienti s mnohočetnou či chirurgickou extrakcí a dále pacienti s poruchou hemostázy na více úrovních.

Abstract (in English)

Most people need some dental care during their lifetime. It might be the point when a bleeding disorder is detected in patients who seemed to have apparently been healthy by this time. In patients with known bleeding tendency this situation can call for good interdisciplinary collaboration. Patients with anticoagulation treatment are involved among those ones with heamostatic problems. Nevertheless, they mostly do not have to interrupt their therapy. They just have to know their actual INR therapeutic level and, they need a stomatologist with appropriate erudition. The situation in patients with a severe haemostatic bleeding disorders is different. These patients need not only special procedure in dental extractions but also a lifelong dental care. There is a group of patients in whom mere outpatient treatment is not possible but in whom a several-day hospitalization is necessary. Those are the patients with haemophilia and inhibitor, patients with multpile or surgical extraction and those ones with multiple haemostasis impairment.
