Abstract
Autoři předkládají základní epidemiologická a klinická data u pacientů s výskytem střevních infekcí vyvolaných toxigenními kmeny Clostridium difficile – CDAD (Clostridium difficile associated diseases). Hodnotí soubor šesti nemocných s touto komplikací v průměrném věku 75,5 roků z období 2009 a 2010. U čtyřech nemocných došlo k vyléčení, dva nemocní zemřeli. Pro rozvoj tohoto onemocnění je rizikové užití antibiotické léčby a to především širokospektrých aminopenicilinů, cefalosporinů a fluorochinolonů.. Pomocí řady opatření omezujících spotřebu těchto antibiotik a pomocí důsledného dodržování hygienických režimů se snažíme pozastavit zvyšující se počty nozokomiálních střevních infekcí v naší nemocnici.
Abstract (in English)
The authors present basic epidemiological and clinical data of the patients suffering from intestinal infections caused by C. difficile. They evaluate the group of six patients at the orthopaedic department and demonstrate one typical case of this disease. Furthermore, the study monitors the epidemiological data concerning the occurrence of the Clostridium difficile toxin in St. Anne's University Hospital Brno (examined by Xpect Clostridium difficile toxin A B, Oxoid). In the period of 2009–2010, C. difficile associated disease (CDAD), accompanied by severe diarrhoea, was diagnosed in six patients treated for orthopaedic infection at the First Orthopaedic Clinic. The positivity percentage of the incidence of C. difficile toxin in St. Anne's University Hospital Brno remained the same during the monitored period, however, the number of examinations increased by 100% and so did the number of positive results as a consequence. Potentiated aminopenicillins and cephalosporins appear as the most problematic group of antibiotics from the point of view of CDAD development. In our hospital, they are followed by fluoroquinolones and lincosamides. It is probably due to the limited use of these antibiotics (- 40%). CDAD was diagnosed in six patients at the Orthopaedic Department during the monitored period. The application of antibiotics was extended in all of them. Two of the patients died of CDAD. The use of all antibiotics for CDAD prevention should be subjected to serious consideration. At the same time, all sanitary measures shall be strictly followed in the hospital.
