Abstract

Článek je stručným přehledem hlavních trendů ve vývoji diagnostických a klinických registrů, které tvoří významnou komponentu informačního zázemí personalizované medicíny, zvláště v oblasti onkologické péče. Práce dále prezentuje uspořádání a obsah sady klinických registrů České onkologické společnosti ČLS JEP, které slouží k hodnocení segmentu cílené protinádorové léčby v České republice. Ukázky dokládají hlavní výstupy dosažitelné z registrů, zejména populační charakteristiku léčených pacientů, hodnocení průběhu léčby a dosažené výsledky včetně analýzy přežití. Závěrem vyplývajícím z provedených analýz je fakt, že komplexní onkologická centra přistupují k této nákladné léčbě svědomitě a aplikují ji správně indikovaným pacientům, kteří z ní vzhledem k celkovému zdravotnímu stavu a ke stavu onkologického onemocnění mohou profitovat. Léčení pacienti v drtivé většině absolvují celou předepsanou léčebnou fázi a dosahují měřitelné léčebné odpovědi. Nedochází k významnému množství progresí onemocnění již během léčby a léčba také není ukončována v důsledku nežádoucích účinků. Verifikované závěry analýzy přežití indikují, že u většiny sledovaných přípravků jsou celkové výsledky lepší než přežití popisované v odpovídajících registračních klinických studiích. Tento fakt lze mimo jiné vysvětlit i přísnou selekcí pacientů, kterým je tato léčba v České republice podávána, neboť kohorta léčených pacientů je u většiny sledovaných přípravků významně mladší, než odpovídá incidenci a prevalenci daného onemocnění.

Abstract (in English)

The paper briefly summarizes main trends in progress of diagnostic and clinical registries, which represent a significant component of background information of personalized medicine, particularly in cancer care. The article further presents design and content of a set of the Czech Society for Oncology CLS JEP clinical registries that serve for the evaluation of targeted anticancer therapy in the Czech Republic. Main outcomes achieved by registries projects are documented, particularly population-based characteristics of treated patients, assessment of therapy progress, and achieved results including survival analysis. It may be concluded that comprehensive cancer centres provide this expensive therapy thoroughly and administer it to right patients, who may have the highest profit regarding their health status and cancer stage. Most of the patients undergo whole administered therapy cycle and achieve measurable therapy response. No significant number of disease progressions is recorded during the therapy and premature termination due to adverse effects is also rare. Verified results of survival analysis indicate that overall results for most of the drugs are often better than described in corresponding registration clinical studies. This fact may be explained, among others, by a strict selection of patients to undergo the targeted therapy, since the treated patients’ cohort is significantly younger in most of the medications than it would correspond to the particular disease incidence and prevalence.
