Objectives: it is still difficult to define vasospasms in patients with severe subarachnoid hemorrhage according to the tissue oxymetry monitoring. The aim of this study is to evaluate correlation between brain tissue oxygen levels and transcranial dopplermetry parameters in patients suffering from severe subarachnoid hemorrhage (SAH). Methods: Patients with SAH from a rupture of cerebral vessel aneurysm in clinical status of HH4 and 5 according to Hunt-Hess scale were enrolled into the study. Brain tissue oxygen monitoring (PbtO2, Licox system) of the area supplied by the ipsilateral middle cerebral artery was performed in addition to standard ICU monitoring. Transcranial dopplermetry (TCD) of the ipsilateral middle cerebral artery was performed repeatedly. The correlation between TCD parameters and parameters of PbtO2 was evaluated. Results: Totally we have enrolled 27 patients, 5 patients were subsequently excluded because of bad position of the probe or early failure of PbtO2 monitoring. In the group of 22 evaluated patients, there were 17 females and 5 males and the mean age of this group was 53,3 years. There were 15 patients in clinical status of HH4 and 7 patients in status of HH5, clipping of the aneurysm was performed in 14 cases, while the coiling was used in 8 patients. The outcome parameters were as follows: GOS 1 (death) - 8 patients, GOS 2 and 3 - 11, GOS 4and 5-3 patients. In our study group there was no significant correlation between Pbt02 and parameters of velocity (p=0,2499, r=0,1998 for Vmean), however there was a significant correlation between Pbt02 and both arterial resistivity index (Rl, p=0. 0061, r=-0.4546) and pulsatility index (PI, p=0,0089, r=-0,4359).
