Bullous pemphigoid (BP) is an autoimmune blistering disease with high morbidity, particularly in the elderly. Conventional treatment is often associated with numerous adverse effects. Recent insights into the pathogenesis of BP highlight the role of Th2-mediated inflammation, opening the door for targeted biological therapies. The presentation describes a case of a polymorbid patient treated with dupilumab, resulting in clinical improvement and corticosteroid withdrawal. Although used off-label, dupilumab appears to be a promising alternative for managing treatment-resistant BP.
