In a place of coloboma there was absent Zonula Zinii in a range of 120 degrees. We used topical anaesthesia with Novesine and neuroleptanalgesia with Sufenta and droperidol. Phacoemulsification with clear cornea tunnel was used. After partial phacoemulsification we introduced intracapsular supporting ring, which enable complete surgery and implant posterior chamber intraocular lens. Result: Surgery was complicated with small vitreous prolaps, which was managed by anterior vitrectomy. Supporting ring held integrity of capsular bag during all session. After surgery patient had a normal visual acuity 1,0, round pupil and well centred intraocular lens. Conclusion: Surgery in-patient with congenital lens coloboma is rather complicated but with intracapsular supporting ring is possible to perform phacoemulsification and posterior chamber lens implantation. The introduction of ring is better to do before complete phacoemulsification. The surgery is simpler in a case of soft cataract. ER -
