Abstract

Mykotické aneuryzma abdominální aorty je relativně vzácnou, však potenciálně maligní entitou, zvláště pokud běží o rupturu. Představuje tak stálý problém na poli cévní chirurgie. Existuje více terapeutických přístupů. I přesto, že některá sdělení upozorňují na úspěchy endovaskulární léčby, zůstává metodou volby terapie těchto pacientů chirurgická intervence, která skýtá několik modalit. Autoři prezentují případ urgentně chirurgicky řešeného pacienta s rupturou mykotického aneuryzmatu abdominální aorty u něhož byl k aorto-biili-akální rekonstrukci použit graft z autologní vena femoralis superficialis. Pooperační údobí proběhlo bez komplikací. Klinický případ potvrzuje kladné výsledky docílené zahraničními autory tímto typem rekonstrukce ve srovnání s jinými alternativami chirurgické léčby my-kotických aneuryzmat.

Abstract (in English)

Mycotic aneurysm of the abdominal aorta is a rare, but potentially lethal disorder because of the risk of rupture. It presents a permanent problem in vascular surgery. Despite recent advances in endovascular procedures, one of the available surgical interventions gene-rally remains the method of choice. The authors present the case of a patient with a ruptured mycotic aneurysm of the abdominal aorta who urgently underwent aorto-biiliac reconstruction by an autologous superficial femoral vein graft. The patienťs post-operative conditi-on with the absence of complications is in agreement with the good results achieved with this technique by surgeons abroad, and gives support to the notion that this approach is superior to other surgical treatments of mycotic aneurysms.
