Course objectives
The subject is drawn up as a follow-up and connection to the theoretical- practical unit together with the subject of Nursing in intensive care II. Contents of this subject lies in the necessary theoretical knowledge, practical skills and activities in the field of highly specialized intensive care nursing which is applied in patient care resuscitation departments and intensive care units. It follows the knowledge and skills acquired in the subjects of anesthesiology and resuscitation and other subjects in intensive care.
Learning outcomes
After graduating from this subject a student will be able: to describe organizational, personal and material equipment of resuscitation departments and intensive care units, to tell the diference between desinfection and sterilization; to define nursing process in resuscitation and intensive care and needs of patients in intensive care; to name the basic tests and scales in intensive care; to define and describe monitoring in the intensive care; to show the chosen aproches connected with nursing care in laboratory environment (taking care of respiration tract, ensuring the cleareness of airways, taking care of secured airways); Furthermore, the student will be able: - to prepare aids for securing central venous access including assistance during the procedure, - to know the right approach for inserting nasogastric tube in patients with secured passage of airways, - to provide aids for punction or chest drainage including assistance during the procedure; - to prepare patients for bronchoscopy or tracheostomy; - to know device equipment in intensive care units and to be able to use them properly; - to know principles of manipulation with syringe pump, infusion pump, pump for enteral nutrition, ventilator, heating devices, monitor, pulse oximeter and other devices, - to know the specifics in oxygen therapy, - to prepare a sterile table for aseptic procedures; - to evaluate monitoring parameters (breathing, blood pressure, pulse, temperature, oxygen saturation in conscious and unconscious patients; Under laboratory conditions students will be able to: - - prepare a ventilator to perform artificial pulmonary ventilation; - connect the patient to the ventilator; - to suck mucus from the airways with open and closed suction systém; - to change position of endotracheal tube; - to monitor and analyze ventilation parameters; - to take blood for examination of blood gas and acid-base balance; - to evaluate patient status during the weaning the ventilator; - to know the correct procedure when disconnecting the patient from the ventilator; - to remove the endotracheal cannula, - to assess consciousness by using monitoring techniques, - to evaluate the depth of analgosedation by using assessment methodologies, - to perform comprehensive highly specialized care for a patient with a disturbed consciousness.
Syllabus
  • - Organizational, personnel and material facilities for resuscitation and intensive care units. Instrumentation for monitoring vital vital functions, diagnostic and therapeutic functions and devices for organ function support. Material equipment of intensive medicine centers. - Documentation in intensive medicine workplaces (nursing documentation, medical documentation, other documentation). - Nursing process in resuscitation and intensive medicine, patient needs in intensive care. Nursing documentation + tests and scales used in intensive care. - Definition, meaning and general objectives of monitoring. Basic Physical Examination, Examination Methods, Analysis of Patient Health Details. - Possibilities of cardiovascular monitoring (pressure, pulse, ECG, hemodynamic monitoring, blood circulation, vascular monitoring). Patient preparation and assistance in the introduction of central venous catheters. Highly specialized care before, during and after procedure. Possible complications of their solution. - Possibilities of respiratory monitoring (respiratory rate monitoring, pulse oximetry, capnometry, blood gas and acid-base balance, monitoring during artificial lung ventilation, and other monitoring options). - Possibilities of central nervous system monitoring (evaluation of quality and quantity of consciousness, depth of analgosedation). Patient preparation and assistance in introducing intracranial sensors. Highly specialized care before, during and after procedure. Possible complications of their solution. - Basic complementary examination, collection of biological material. Indication for examination, types of biological material taken. Proper procedure for collection and handling of biological material taken. Physiological values of individual examinations. - Nursing care for the respiratory tract. Problem of cessation from the ventilator. Extubation of the patient. Complex highly specialized care for patients with artificial pulmonary ventilation (care for the endotracheal and tracheostomy cannula, respiratory tooth, suction, respiratory rehabilitation, endobronchial lavage). - Intensive and specific nursing care for the respiratory system. Highly specialized nursing care for patients with severely impaired respiratory system. Care for patients with artificial pulmonary ventilation. Patient preparation and assistance in the introduction of operative and dilatation tracheostomy, chest drainage, bronchoscopy. Highly specialized care before, during and after procedure. Possible complications of their solution.
