Course objectives
The care for permanently growing number of the elderly, which are currently multi-morbid and deteriorated in the psychic and social sphere, will be more demanding not only in terms of the system of the organization of the health and social care but also due to increasing requirements of higher degree of knowledge and skills both with the professionals and all other care providers. It is not just the question of assistance for the patients, it will be more and more necessary to understand them and their specifics in diagnostics, therapy and overall care in medical and social institutions and facilities. Present reality requires complete change of the elderly population awareness both in the professional community and in the general public, which is also one of the tasks for gerontological education. Geriatric patient is special since their etiopathogenetic diagnosis made with classical diagnostic biomedical methods does not say anything about the quantitative extent of required care, dependency on the environment and quality of their further life. The influence of any disease present is much more significant in old age than with younger patients as long as management of independent life is concerned. The overall treatment of any disease in the elderly should be necessarily focused not only on the causing disease but also on maintenance (or contingent improving) of self-sufficiency, functioning social connections and relationships in the families and community. Geriatric patient should be seen as a “frail” senior with limited (or mostly decreased) functional capacity and impaired adaptability. Such person is long-term threatened by decompensation of the functional status and needs specific modification of professional attitude to the regimen and therapeutic measures. The subject is aimed at offering a useful manual for daily work and activity to the future graduates of the master degree studies (medicine, nursing, social workers and other professionals caring for the elderly people). The tuition is focused not only on enlargement of the professional knowledge and skills, but also on higher level of understanding the elderly in the medical, psychological and social sphere. The main effort of the seminars and lectures is to introduce to the students questions and problems which they will come across most likely very often in contact with the elderly such as the so called geriatric giants: immobility, instability, incontinence and intellectual impairments. They are closely connected with decubitus, falls, delirium, dementia and depression. Problems of the general and special geriatrics are discussed according to the organ systems. The subject is by no means understood as a mere overview of gerontology and geriatrics, on the other hand it does not claim to be a full explanation of very complicated comprehensive clinical picture of the diseases and clinical syndromes. In a limited framework the contents of the subject reflects modern pieces of knowledge from recent years in epidemiology, etiology, pathogenesis, clinical picture, prognosis and therapy, including their social consequences such as handicapping and disability resulting in the need of assistance and care.
