Kapitola pojednává o farmakorezistentní depresi, její definici, klasifikaci a faktorech s ní spojených, dále o diferenciální diagnostice a konečně o léčebných možnostech, které jsou rozděleny na farmakologické a nefarmakologické. Mezi farmakologické postupy jsou řazeny prodloužení léčby a zvýšení dávky, změna antidepresiva, augmentace antidepresivní terapie a kombinace antidepresiv, mezi nefarmakologickými postupy jsou uvedeny elektrokonvulzivní terapie, repetitivní transkraniální magnetická stimulace, stimulace nervu vagu a také psychoterapie.
