Course objectives

The aim is to teach students the basics of the physical examination of patients in surgery and internal medicine.

Learning outcomes

After completing the subject Propaedeutics the student will be able to take complete patient history, to perform general physical examination of the patient, to perform basic internal, neurological and surgical examination of the patient. Student will be familiar with basic laboratory and other diagnostic tests. Student will be able to perform and evaluate the ECG, will be able to recognize basic pathological ECG findings. He will be able to describe the physiological findings on chest and abdominal X-rays. He will be familiar with basic surgical instruments together with their use, will be familiar with immobilization technique, with basic technique of sewing, punching, cannulation, preparation, catheterization.

Syllabus

A student completes 7 hours of practical exercises weekly, divided into two instructional sessions of 3.5 hours each. One instructional block focuses on internal medicine, supplemented with a rotation at the Anesthesiology and Resuscitation Clinic. The second block focuses on surgery, with one rotation in pediatric surgery and one in plastic surgery. During the 13th and 14th weeks, instruction takes place only in SIMU, where the focus is on practicing practical skills.
• **Internal Medicine:** Instruction is conducted in study groups, with the following topics for each rotation: 1. Basic clinical and auxiliary examinations, laboratory, imaging, and functional diagnostic methods (X-rays of the chest and abdomen, principles of ultrasound, fibroscopy, and other auxiliary examinations). 2. Special medical history and examination for pulmonary diseases. 3. Special medical history and examination for heart diseases. 4. Basic pathological patterns on ECG curves. 5. Special medical history and examination for kidney and urinary tract diseases. 6. Special medical history and examination for gastrointestinal and liver diseases. 7. Special medical history and examination for endocrine diseases and diabetes. 8. Special medical history and examination for hematological diseases. 9. Special medical history and examination for peripheral artery and vein diseases. 10. Special medical history and examination for rheumatological and immunological diseases. 11. ARK/KARIM/KDAR – General and local anesthesia, the role of the anesthesiologist/workplace. 12. Review. 13. and 14. SIMU – Practicing practical skills. 15. Review.
• **Surgical Part:** Instruction is conducted in study groups, with the following topics for each rotation: 1. Metabolism and nutrition in surgery – Enteral and parenteral nutrition, diets, fluid balance, replacement solutions, internal environment – ions and acid-base balance in surgery. 2. Infections in surgery – Wounds, GIT, urinary, respiratory, sepsis, tetanus, rabies, antibiotics in surgery, purulent diseases of the hand. 3. Injuries to joints, muscles, tendons – Open and closed injuries, characteristics, diagnosis, treatment principles. Wound care – Review. 4. Nerve and vascular injuries, hemorrhage control, blood loss replacement, blood transfusion, shock and its types, drowning, hanging, electric shock injury. 5. Fractures – Healing and treatment, fracture healing disorders, rehabilitation, prosthetics, forensic assessment. 6. Surgeries – Types, indications, preoperative preparation, postoperative response, intraoperative and postoperative risks and complications, prevention of venous thromboembolism, antibiotic prophylaxis. 7. Basic surgical approaches and procedures: Approaches – Head, thoracic cavity, abdominal cavity, pelvis (including scopic approaches), limbs, basic surgical procedures – Terminology. 8. Trauma surgery – Polytrauma, combined injuries, mass casualties, trauma plan, war injuries (triage and evacuation of the wounded + treatment principles) including blast syndrome, crush syndrome. 9. Oncologic surgery – Tumor classification and spread, prevention, organization of oncological care. Introduction to transplants. 10. KPECH – Plastic surgery – Scope of plastic surgery, flaps, treatment of traumatic amputation, frostbite, burns. 11. KDCHOT – Pediatric surgery – Specifics of pediatric patients. 12. Review. 13. and 14. SIMU – Practicing practical skills. 15. Review.

Teaching methods

Practical teaching of patient bedside care in clinics (in the scope of about 50% of the total volume of Propedeutics I to III) is supplemented by a comprehensive spectrum of simulation teaching methods on simulators with varying degrees of fidelity, simulators and virtual patients. Simulation teaching culminates in a debriefing (feedback to the student). Problem-based learning, where the student learns by solving a presented problem, is also at the forefront, as well as team-based learning, where students discuss and select a solution to a given problem together in smaller groups. Emphasis is also placed on the development of soft skills, including "21st century skills", i.e. in particular communication, decision-making skills, critical thinking, crisis communication and teamwork.

Assessment methods

Students' knowledge and skills are continuously monitored. Students must meet attendance requirements at each clinic and v SIMU. The course is completed by an examination, which has a practical part that takes place in SIMU (OSCE exam) and a theoretical part that is divided into internal and surgical. The theoretical parts of the exam are conducted at the individual surgical and internal departments by appointment.
